As for starting a program with a condition rather than with a block, I’m not really sure how to do this. I’m pretty sure you always have to initiate the program first (with the orange button).

Yes, you can do this. The easiest way is to use a special wait block that uses the ultrasonic sensor as its control. You can configure it to wait to execute other actions (such as motor actions) until the sensor has detected an object within a range you specify.

The NXT-G software (version 1.1) will be supplied with any new retail version of the Mindstorms NXT kit. If your NXT kit is older and came with v1.0, you can purchase v1.1 from Lego Shop website for about 8 dollars.
